This fundamental difference makes it the gold standard for achieving clear vision in individuals who would otherwise be disqualified from laser vision correction.

What is the Cornea and Why is it Important for Vision?
Before we delve into the specifics of laser surgery, it's essential to understand the part of the eye we are treating.
Think of your cornea as the transparent front dome of your eye, much like the crystal on a watch.
It's a remarkable piece of biological engineering, responsible for about two-thirds of the eye's total focusing power.
The cornea's primary job is to bend, or refract, incoming light so that it focuses precisely on the retina at the back of the eye.
For you to see clearly, this dome must have a smooth curve and be perfectly transparent.
Any irregularities in its shape or clarity can lead to refractive errors like myopia (nearsightedness), hyperopia (farsightedness), and astigmatism.
Laser vision correction procedures like PRK and LASIK work by subtly reshaping this dome, correcting the way it bends light and thus eliminating the need for glasses or contact lenses.
The precision required for this reshaping is extraordinary, measured in microns (a micron is one-thousandth of a millimetre).

What is a Thin Cornea & Why is it Important for Laser Vision Correction?
The average human cornea has a central thickness of about 540 to 550 microns. However, this can vary significantly among individuals.
A cornea is generally considered "thin" if its central thickness is less than 500 microns. During a pre-operative evaluation, this is one of the most critical measurements we take.
Why is thickness so important?
Every laser vision correction procedure involves removing a small amount of corneal tissue with an excimer laser.
The amount of tissue removed depends on the power of your prescription—higher powers require more tissue ablation.
The crucial rule in refractive surgery is that a certain amount of corneal tissue must be left untouched to ensure the eye's long-term structural integrity.
This untouched tissue is known as the residual stromal bed (RSB).
The globally accepted standard is to leave an RSB of at least 250 microns, though many surgeons, myself included, prefer a more conservative figure, often aiming for 300 microns or more for an added margin of safety.
In my practice at Envision LASIK Centre, I frequently encounter patients who are perfectly healthy in every other respect but have corneas measuring, for instance, 480 microns.
If such a patient has a high prescription, performing LASIK could compromise the RSB, increasing the risk of a serious condition called post-LASIK ectasia.
This is a progressive thinning and bulging of the cornea that leads to distorted vision and can be very difficult to manage.
Therefore, corneal thickness is a non-negotiable safety parameter.

How Do PRK and LASIK Compare?
While both PRK and LASIK use the same excimer laser to reshape the cornea, the way they access the treatment area is fundamentally different.
This difference is what makes one suitable for thin corneas and the other not.

Why Is PRK Safer and Better for Thin Corneas?
The core advantage of PRK in the context of a thin cornea lies in one simple fact: no flap.
Let's do some simple mathematics.
In LASIK, a flap of approximately 110 microns is created.
This flap, once repositioned, does not contribute significantly to the biomechanical strength of the cornea.
So, if we start with a cornea of 490 microns, after creating the flap we are effectively working with 380 microns (490 - 110).
If the patient’s prescription requires the laser to ablate 80 microns of tissue, the residual stromal bed (RSB) would be 300 microns (380 - 80).
This is on the cusp of the accepted safety margin.
Now consider PRK for the same patient.
We start with the same 490-micron cornea.
The epithelium, which is about 50 microns thick, is removed and will grow back.
It is not part of the structural stroma.
The laser then ablates the same 80 microns of tissue directly from the surface. The total stromal thickness remaining is 410 microns (490 - 80).
The result: PRK leaves the cornea 110 microns thicker and structurally stronger than LASIK would for the exact same patient.
In my 10+ years as a surgeon, I've seen the relief on the faces of patients who were previously told they were unsuitable for laser correction.
A young software engineer from Hyderabad recently visited my clinic, disheartened after being rejected for LASIK elsewhere due to his 475-micron corneas.
He had a -6.00 diopter prescription.
Performing LASIK would have been unsafe.
By opting for PRK, we were able to safely correct his vision, leaving him with a very healthy residual bed.
His journey through the initial discomfort was a small price to pay for a lifetime of clear, safe vision.
This is a classic example of why PRK is not a "second-best" option but a highly specialized & best treatment for the thin cornea.
How Do I Decide the Best Procedure for My Patients at Envision LASIK Centre?
Choosing between PRK, LASIK, or another procedure is one of the most important decisions we make together.
It is a highly personalized process based on a comprehensive evaluation. Here’s a look into what goes through my mind when I assess a patient:
1.Corneal Topography and Tomography: This is the first and most critical step. We use advanced diagnostic machines like the Pentacam to create a detailed 3D map of the cornea.
This tells me not just the central thickness, but the thickness across the entire cornea, the shape of its front and back surfaces, and flags any hidden irregularities like early-stage keratoconus.
2.Refractive Error (Your Prescription): The higher your prescription, the more tissue we need to ablate. I calculate the predicted ablation depth and subtract it from the corneal thickness to determine the final RSB for both PRK and LASIK.
3.Pupil Size: In dim light, your pupils dilate. We measure your pupil size in a dark room to ensure the laser treatment zone is large enough to prevent night vision issues like glare and halos.
4.Tear Film and Dry Eye Assessment: LASIK can sometimes exacerbate dry eye symptoms, at least temporarily, because the creation of the flap severs some corneal nerves. Patients with pre-existing significant dry eye might be better candidates for PRK, which has a lesser impact on these nerves long-term.
5.Lifestyle and Profession: For patients in professions with a high risk of eye trauma—such as military personnel, police officers, or professional athletes—I often recommend PRK. Because there is no flap, there is zero risk of flap-related complications from an injury, even years down the line.
6.Patient Temperament and Expectations:
I have a frank discussion with every patient about the recovery process. A patient who needs to be back at work with perfect vision in 48 hours might be psychologically unsuited for the slower visual recovery of PRK. We must align the choice of procedure with the patient's lifestyle demands and their ability to tolerate the recovery timeline.
What Can You Expect During and After PRK?
Understanding the recovery process is key to a positive PRK experience.
The Procedure Itself:
The surgery is quick, lasting only about 10-15 minutes for both eyes.
Your eyes will be numbed with anaesthetic drops, so you won't feel any pain.
I will gently remove the epithelial layer, and you will be asked to focus on a target light while the excimer laser precisely reshapes your cornea.
This laser portion typically lasts less than a minute per eye.
The First 3-4 Days (The Healing Phase):
This is the most challenging part of PRK recovery.
Once the numbing drops wear off, you will experience discomfort, a gritty or foreign body sensation, light sensitivity, and watering.
Vision will be blurry. I place a special 'bandage' contact lens on the eye to protect the healing surface and manage discomfort.
It’s crucial to rest your eyes, stay in a dimly lit room, and use the prescribed medicated eye drops diligently.
1 Week & Follow Up:
I always tell my PRK patients to clear their schedules for at least 3-4 days post-surgery & compulsory come for follow up.
Try limiting to work or use screens during this time is not only uncomfortable but can also hinder the healing process.
Patients who fully commit to resting during this initial period report a much smoother and faster recovery.
1 Week to 1 Month:
The bandage contact lens is removed after about 3-5 days, once the epithelium has regenerated.
Your vision will be functional, but it will continue to fluctuate. You may notice some haziness or glare, especially at night.
This is normal as the corneal surface remodels.
1 to 3 Months and Beyond:
Your vision will become progressively sharper and more stable.
The final, crisp 20/20 outcome is typically achieved within 1 to 3 months, although minor refinements can continue for up to six months.
Most Common Questions About PRK Patients Ask
1.Does corneal thickness increase after PRK?
No, the corneal thickness does not increase after PRK. The structural layer of the cornea, the stroma, that is reshaped by the laser does not regenerate. However, the outermost layer, the epithelium, which is removed during the procedure, does grow back to its original thickness of about 50 microns. This epithelial regeneration is a key part of the healing process but does not add to the cornea's overall structural strength or thickness.
2.How long after PRK can I see 20/20?
This is one of the most common questions, and the answer varies. Many patients achieve or are close to 20/20 vision within the first month. However, for the vision to become stable and consistently crisp, it can take anywhere from 1 to 3 months. The key is patience. Unlike the "wow" effect of next-day vision with LASIK, PRK is a gradual journey to excellent vision.
3.Does the epithelium regenerate after PRK?
Yes, absolutely. The human body's ability to heal is remarkable. The epithelial cells at the edge of the treatment area begin to multiply and migrate to cover the exposed surface within hours of the surgery. A complete, smooth new layer is typically in place within 3 to 5 days.
4.How do I know my cornea is healing?
The initial signs of healing are a reduction in pain, tearing, and light sensitivity. As the days go by, the primary indicator is the steady improvement in your vision. I look for a smooth, uniform epithelial layer and check for any signs of haze or inflammation, ensuring your healing is proceeding exactly as it should.
